PK/PD with Monte Carlo simulation of isoniazid in tuberculosis patients

Introduction:

The therapeutic relationship between isoniazid (INH) plasma concentrations and tuberculosis (TB) treatment response has not yet been defined in human studies. The recommended 2-hour target range for INH is 3-5 mg/L. An animal study suggests that AUC24/MIC best describes the bactericidal effect of INH, and that an AUC24/MIC of 500 h is required for maximum efficacy of INH.

The present study reports the results of a Monte Carlo simulation based PK/PD analysis of INH in TB patients receiving standard TB treatment of INH, rifampicin, ethambutol and pyrazinamide once daily. The PK/PD analysis was performed to determine probability of target attainment for AUC24/MIC = 500 h.
